Good Day, I Was Clearing A Vine Out Of My

Good day, I was clearing a vine out of my yard Monday (San Jose, CA) and believe I was bitten by a spider. The bites itched yesterday am, and i used an OTC cortisone ointment. Today the area-forearm near elbow-has an area about 1" diameter that is swollen about 1/2 " high. There is no obvious pustule, or fluid draining, but the area is a firm lump, and still itches. Should I ice area & continue to use cream?
