Grandson Aged 24. Drug User Since Age 12. Attempted Suicide

Grandson aged 24. Drug user since age 12. Attempted suicide (overdosing) five years ago. Some brain damage from that experience, apparently permanent, chiefly affecting memory and ability to plan ahead. Immature; seems to be the equivalent of a 15 yr. old. One year-long prison term for assault during an attempted robbery (released last Jan.). Now an opiate/heroin addict, in and out of rehab. u
Unable to find/keep minimum wage employment. Family hard-pressed financially (beyond inadequate insurance coverage) to keep paying for what looks like palliative care. Now back home. RN mother administering daily RX to quell urge to find heroin (do not know name of medication - not methadone).
Question: How to find a long-term care facility, possibly in the back of beyond (Idaho? Wyoming?) that might involve the kind of physical activity that would assist in restoring health and provide a way to achieve minimum self-reliance? For the "best" treatment, what would the likely per annum cost be?
