Having Dealt With GERD Most Of My Life, Also Already

Having dealt with GERD most of my life, Also already having had a Nissen Fundo about ten years ago. About 2 years ago I developed chest pains and after a second EGD it was founf my Nissen had loosened and I had to go back on PPi's again which worked for over a year. Now I've developed a cough that has lasted about six months and now I have chest pains and food sometimes gives me a problem going down. I also don't have any appetite. I've also been using 9-12 ibuprofen a day for over six months dealing with a torn rotator cuff that I've had fixed for weeks ago. I haven't used any ibuprofen since but the symptoms aren't going away. Can this be more serious that esophagitis even though I've had an EGD 1 year and 10 months ago? I've been on PPI's all along.
