Hello Actually There Is Rough Skin On My Penis As

Actually there is rough skin on my penis as well as on the testicals...i have problem of white patches on the skin so my doctor has given me the cream named "placenta extract gel"
And i was applying this on the skin and it has become very dry
My 2nd question is that-my foreskin has become tight and doesn't easily go back and causes pain
Please help!!!
