Hello Doctor, I've Got A Question Regarding My Speech/communication Which

Hello Doctor,

I've got a question regarding my speech/communication which has been affecting my life the past 7 years or so.
I seem to lose words and find it very difficult to speak some days whether its with new people I've met or very close friends and family. My tongue feels very num and swollen during the indecents and I almost feel mute.
I've also been taking medication for thyroid the past 7 years.
