Hello: I Am A 55 Year Old White Female, ½

Hello:
I am a 55 year old white female, ½ pack per day smoker X 30 years, 215 pounds, 5’4”. I take meds for high blood pressure, depression, acid reflux, sinus meds. Another words, not in that great of health and have no one to blame but myself.
My issue is that I am kind of worried and maybe reading a little too much on the internet, (Trying to be a doctor, so to speak) I have worked in the health care field for over 25 years, mainly in the clerical area.
Back in 2007-2008 I worked at a automotive plant making shocks and struts. I have to admit I loved the job, people. Was probably in the best physical shape of my life, actually had abs,ect. Needless to say, that job was temporary and soon the plant was closed.
I have a few issues going on. Approximately 1 ½ years ago, I noticed bilateral humerus pain between the shoulder and elbow area after lifting some heavy boxes. The pain comes and goes, depending on weather, activity, ect. I take over the counter meds for the pain. It is not muscular, but more deeper, like bone pain. Ben Gay, heat does not help. Pain increases with working overhead. Some days it is a struggle to lift my purse.
I have mentioned it to my PCP and she mentioned it might be a torn rotator cuff and to go see a ortho doc. I have not done so because I have a very high co-pay and my insurance sucks, especially for imaging and therapy. I feel like it went in one ear and out the other. That was about a year ago.
Now, this is where I am starting to get scared. In looking back on my history, I’m starting to question some things.
In October of 2014, I had a regular screening mammogram. I have never had an abnormal one, but got called back for additional views this time. They were looking at a spot in the twelve o’clock position. It did come back negative at that time.
About 2-3 weeks ago I noticed that I had a red circular patch of skin about the size of a quarter on my left breast at the 2 o’clock position. I went to my PCP and she said it looked like ring worm and prescribe me some cream. She ordered a mammogram, because I was due for one. I had that done 1/7/16 and have still not heard from them or my PCP.
Of course I start to look at the internet and keep stumbling on IBC and bone mets. Could my humerus pain be bone mets? Another thing. My maternal grandmother died of multiple myeloma at the age of 63. Could this be related, too.
If the mammogram is normal should I insist on a biopsy of the patch on my breast? Please help.
