Hello, I Am A Female Health Care Provider, Turning 44

Hello, I am a female health care provider, turning 44 next week with 3 kids. I had awful weekly migraines throughout college and it has gotten better over the years now to about 4 per year. However, I have chronic pain that is getting worse especially in the last 2-3 months so that now it is 100% of the time, interrupting my sleep, and at about level 8 constantly. The part that has worsened is the ringing of the ears, level of pain, and there are more distinct areas additional to where it used to "just hurt all the time". The usual areas were my entire back half from shoulders to my back of my arms to elbows and back to legs to about the calves, symmetrically. I will not take medication and do not want surgery, but would like to know what tests to start to rule out things I can solve. I have a spur at c4-c5 left side that was caught soon after an automobile accident almost 15 years ago, but am uncertain if that is the exact trigger. Please advise what type of doctor will get to the root faster but with natural cures if possible. THanks!
