Hello I Am Very Embarrassed To Ask A Specialist For

Hello I am very embarrassed to ask a specialist for help in person. I use to sniff cocaine and now have a very large hole in my nose. It gets very crusty and unfortunately I have "picked" at this until it bleeds. What are my options here? Do I have to say what I think has caused this hole, or will the specialist just know??
