Hello, I Have Had Pain After Smoking Cigarettes Lately.. A

Hello, I have had pain after smoking cigarettes lately.. A tightening of what I think is my heart, a little bit of left arm nerve pain. if I don't smoke, I am not in pain. I have been wearing the patch and if I cheat and have a smoke, I have a bit of pain in the heart. Should i be concerned?
Since I am on birth control, I worry these may be symptoms of a clot?
