Hello. I'm submitting this form for Riley Fairbert, male, age 22. We would like to go through OSF in Rockford, IL to get a new psychiatrist for him. He has been diagnosed with Bipolar II and Panic Disorder in the past couple months. This is the first time he has reached out for treatment. He had a severe "black box" reaction to two medications on two occasions and was in the ER for suicidal ideations multiple times. His blood pressure gets so high from his anxiety it can hit 147/120 with a heartrate of 117 (like it did today). A qualified psychiatrist with an MD, preferably who specializes in panic disorders, would be his best option.
