Hello. My 86 Year Old Grandma Who Has Sarcoidosis And

Hello. My 86 year old grandma who has sarcoidosis and congestive heart failure was released monday after a 3 day visit at Little Rock Heart Hospital where they installed a pacemaker. I then took her to OCMC due to strange behavior where it was discovered she had UTI. She was released yesterday and today she is nonresponsive where as I cant give her medicine, updrafts, food or drink. I've checked her vitals and changed her diaper but how am I to administer her care? Why would they release her knowing it would be this way until the infection is completely gone?
