Hello My Father In Law Collapsed And Foamed Through His

My father in law collapsed and foamed through his mouth and whilst taking him to hospital he passed. Just wondering what might have caused this.. Just can't understand as he was discharged from hospital And was fine.

My father in law had a heart attack around 10 days ago and was in hospital, they put him on a lots of medication.(beta blocker/blood pressure/acid reflux/ sleeping tablets) they did a angiogram before discharging him. Angiogram came back fine no blockage was found. We are aware he has heart disease and a weak heart. He had a heart attack around 7 years ago which happened to be a full on one. This one hes had now is his 2nd but wasn't as strong as his first.

Around 4 months ago he wasn't well his diebetes was up and down so they changed his tablets and gave him insulin to inject instead. Anyway this was given as instructed and after around 3rd or 4th day they found him in the morning foaming through the mouth and unresponsive. So rushed him to hospital. Doctors said he's diebetes level is very very low that's why he was in this state. Anyway he recovered from that and he was told to take the tablet form to control his diebetes.which he did and was good for few months untill he had his heart attack.

Now going back to his current heart attack 10 days ago. Doctors gave him along with medications mentioned above insulin,one a fast acting one and one long lasting one. I'm wondering could the insulin have caused him to collapse and foam through his mouth or could he have had a another heart attack.
