Hello, My Brother Is In The Death Process Due To

Hello, my brother is in the death process due to stage 4 saliva gland cancer. We were also informed that he recently has a tear in his trachea (I may be mis-spelling this word). What would cause a tear in the trachea? I realize that nothing can be done for him at this point...I'm just curious. Thank you.
