Hello, My Next Door Neighbor Who Is 53 Yrs Old

Hello, my next door neighbor who is 53 yrs old has noticed his testicles have turned black and blue for some unknown reason. he happen to be an alcoholic who started drinking at age 14. Could this be the cause? What should he do. He is afraid to see a Dr. about any and all health issues.
