Hello There. I Have Questions Regarding My 63 Year Old

Hello there. I have questions regarding my 63 year old mother. She has been smoke free since January of 2015 after 40+ years of smoking. She has been diagnosed with Diabetes,Asthma and COPD. Well CHF landed her in the hospital 3 weeks ago. Through numerous tests she has now been diagnosed with sleep apnea, Stage 3 kidney disease and heart disease. Her EF is 15% and she is battling back and forth in A Fib. The doctors say that in this case a heart transplant is needed but since she is overweight (241lbs) and other factors take her off of the donor list. Why? What is her life expectancy? How can be extend her quality of life?
