Hello.....I Have A Blood Clot In My Lungs And On

Hello.....I have a blood clot in my lungs and on blood thinners for almost a year now. I'm just so tired, and not feeling well most of the time....I'm taking warfarin. What should I do, and how long should I take this medication. My legs are a mess only my left leg and I do get some swelling in that leg.
