Hi Doctor, I Am 67 Yerar Old Male,dabatic Patient For

Hi Doctor, I am 67 yerar old male,dabatic patient for 25 years Last 7 months I foyund my S.Creatine level is 3.6 mg.dl and now it is increased to 4.5 mg/dl.I went to see a doctor(Nephrologist) and he said that it is Anemia-stage-IVkandCKD-IV stage.(Diabatic Nephropathy). Blood Urea is55.
Now I feel tired and some times getting Respiratory Problems and Stomach disorder.
Is it possible to reduce my S.Creatine and other sickness?
Like to get good adcvice and treatmentr
