Hi Doctor, My Child Whose Age Is 2 Years And

Hi Doctor, My child whose age is 2 years and 9 months is suffering from high grade fever. Since 10th Nov. afternoon it started and it goes down with Panadol to 98F and then get back to 102F. The Panadol was given 7.5 mlevery 6 hours.
Yesterday we started ibugesic plus which consist of ibrufen and paracetamol since the fever was not coming lower. We have her 3 doses yesterday but the fever reached 104F plus so we visited Doctor who has checked and said it looks like high grade fever as her chest and throat was clear.
The doctor have Crocin suspension DS and the fever cam down but again went up within 4 hours to 101F plus.
What should we do. Should we go for a blood test or still wait and watch as doctor has asked to monitor for 48 hours
