Hi I Am A 44 Year Old Woman And About

Hi I am a 44 year old woman and about two and a half months ago I had a hemorrhoidectomy. They surgeon condensed down the size of the rectum passage and now my bowels have to remain soft as possible. To make a long story short,, one day it was not soft and I tried to strain to get the bowels to pass and they wouldn't. In my straining I felt a sharp like pulling pain in the sternum area and on the right side of my upper chest. Every since then my chest has been extremely painful. If I bend forward, lay on my chest or lift my arms up, cough or anything. Do you think I tore something? Please advise me
