Hi, I Am A 52 Y/o Male 305 Lbs With

Hi, I am a 52 y/o male 305 lbs with diabetes 2, high blood pressure, and high cholesterol with a stent inplant as of april 2016, currently taking Amlodipine 10mg once daily, Brilinta 90mg BID, Metoprolol Tart 25mg BID, Lisinopril 20mg once daily, and Coreg CR 40mg once daily. and my blood pressure is low in the day and spikes in the evening Low is arounf 76/58 paulse 108 and Spike is 159/101 Paulse 121 I have several ER visit in the evening. How should I take these medicine or what should I do?
