Hi, I Had A Skin Graft Taken From My Thigh

Hi, I had a skin graft taken from my thigh three years ago. It's faded a lot and you almost can't even tell that it's there. I'm wondering why it often becomes itchy and forms welts or lumps all over it? Is there anything I can use to avoid this from happening?
