Hi, I'm A 30 Y/o Male And I've Been Bleeding

Hi, I'm a 30 y/o male and I've been bleeding after any sexual activity including masturbation, when I go to urinate there are blood clots in my urine. I have been to the hospital and tested for STI's which all came back negative as well as a ultra sound on the testicles. I have also have very bad pain in my testicles and frequent need to urinate, and now there are blood clots in my urine whether or not I engage in sexual activity.
