Hi, I've Had For About A Year Like Many Pimples

Hi, I've had for about a year like many pimples on my balls and some get really big and are like impossible to pop, some are smaller and I sometimes manage to pop them but when they come out its like when the pimple explodes, they white chesses string comes out, what should I do? Worry about it or leave it alone?
