Hi! My 98 Year Old Grandfather Was Just Recently Released

Hi! My 98 year old grandfather was just recently released from the hospital back to his care home in the more intensive are part. He went in for pneumonia (again) & an enlarged prostate. They are not sure he will be going back to his apartment in the care home. He has been taken off all medications as they are now affecting his kidney's. He is very weak, tired & his legs are swelled up. Unfortunately we have not gotten a straight answer as what to expect or if these are his last day's. Any insight would be very helpful. As his granddaughter who lives far away from him, I know no-one can pin point a time of death but does this mean he is close? I would like to go home to be there for him before it happens so I can say goodbye. But I have a job & kiddos I have to worry about too. So I am concerned about when I should be taking this more seriously & getting there asap Thank you
