Hi I Have A Problem When I Urinate The Last

Hi i have a problem when i urinate the last 3-4 months i see a bit of blood on the paper also some of the paper stays on there and i have to clean myself everytime. Occasionally the area feels a bit itchy and uncomfortable but not in high levels. What should i do? I would say that the skin down there is irritated and kind of fragile?
