Hi My Inner Foreskin Has Very Small Tears During Intercourse...I

Hi my inner foreskin has very small tears during intercourse...I tore the falabrum over 15 years ago...but it healed without surgical intervention. Now my foreskin is harder to pull down past the head although it does after time and some discomfort.
But it seems to have a tighter ring of skin when fully pulled down like a whitish belt. Mehta can I do as it is ruining sexual activity
