Hi, My Sister Has A Very Bad UTI, She Says

Hi, my sister has a very bad UTI, she says it hurts so bad when she urinates and severe lower back pain. She screams in pain she said. And she also said there's a rectal pressure. My question is, are all those symptoms relate to a urinary tract infection. Could it be something else along with the UTI.
