Hi, My Uncle Aged 46 Now Had Gone Through Valve

Hi, my uncle aged 46 now had gone through valve replacement in 2012.Two mechanical valves were put up and one valve was repaired. For the past 3-4 days, urine had stopped with swelling in the stomach.went to a doctor, got echo, abdomen usg, fluoroscopy done.. Valve is working fine but there is mild pleural and pericardial effusion with fatty liver and the heart has also enlarged a bit.. Tests for hepatitis is negative. As per gastroenterologist,, kidney is also fine.. Facing problem in the night, palpitation, difficulty in breathing in the night..
What to do??
