Hi There, I'm A 29 Year Old Female. I've Had

Hi there, I'm a 29 year old female. I've had episodes of painful urination 3 to 4 times a year for 3 days to a week at a time. They are more often then not around the time of my period. Two of the times I went in to see if I had a UTI and the cultures came back negative but I was put on antibiotics anyway. The other times the pain went away on its own in a few days. Sometimes I have a small amount of blood and small white chunks of tissue or cells that pass. I have no pain anywhere besides right at/around the opening to my urethra. The pain is the worst at the end of my urine stream and sometimes persists for hours after I have urinated. The pain sometimes wakes me up at night. The current episode I'm having has lasted for more than a week, some days are more painful than others, and as soon as I feel like it's getting better or almost gone it comes back. Should I be concerned about bladder cancer, could this be related to the Skene's gland, is there something happening with my urethra? Not sure if I should see a gynecologist or a urologist.
