How Can I Get Rid Of A Persistent Skin Parasite Infection With Black Fibers From My Skin?

I have a parasite skin infection which is proving difficult to get rid of perthimin cream has failed 4 times now despite strict cleaning routine of house and body. Also have what appears to be bits of black fibres coming from skin primarily on my hands. What is it and how do i get rid.
