How Can A Foreskin Lump With Splitting Be Treated?

hello my husband is 43 and his foreskin has got very tight and he cant pull it back he is washing all the time and using cream but the skin is now splitting where the foreskin is attached to his penis there is a little lump he is getting very worried can u please give him some advice thanku
