How Can An Occipital Headache With Back Pain Be Treated?

I've been having consistent headache at the back of my head (base of skull) and pain at the middle of my upper back which does not seem to be affected by massage and cannot get my chin to touch my chest. What could be going on? I may also be feeling a little weaker on my right shoulder/arm but no tingling or loss of feeling. No fever.
