How Can Black Spots On The Neck Be Treated?

Hi i have got black spots around the neck.. it is there for many years now. i was under the impresssion this could be due to the gold chain i am wearing. could you please advise whether this is bcoz of any serious problems like diabeties or skin related problems.
What should i do to get rid of this.
