How Can Diabetes Be Kept Under Control?

I m a Type II diabetic currently taking metformin 1000 BID. glimperpride ? 5 mgs BID, and Byetta 5 mcg injections BID, doing Cardiac Rehab 30 minutes on the bike and 25 minutes on the tread mill. since having a slight MI in June CABG 21 years ago because of dissected RCA during attempted angioplasty. and yet my Aic has gone from 7.3 to 7.8, T Chol 145 to 285, Tri 200- 585. weight gain of 10 lbs, and BP elevation. Lopressor 25 mg BID Also on fenofibrate 160. DC pravastatin and Lipitor due to stage 3 bridging fibrosis developed while on those two. No drinker, non smoker, non IV drug user. Also 137 mcg levirothroxine. Can you suggest anything to bring my problems under control.?
