How Do I Better Describe My On Going Debilitating Symptoms

How do I better describe my on going debilitating symptoms to my Doctor? I honestly feel that will will not live much longer at all. My symptoms have gone on for several years now and have multiplied to the point that I'm so frustrated and so depressed. I'm desperate for answers. My family is frightened now also for my life. They have watched me go from a beautiful vibrate young woman to a saddened sick shell of s soul. Please help or tell me where I could get help. I'm so sick. Thank you, God Bless
