How Do I Get My Name Taken Off Your Mailing

How do I get my name taken off your mailing list? I'm unsubscribed five times; contacted the website directly and I was assured my name would be removed shortly. Ten days later I'm still getting emails and I'm very frustrated. This is NOT good PR for your brand.
