How To Cure Chest Pain And Breathless Despite Being On Pacemaker?

hi i have a question about my Grandfather, He has only 20% of his heart working with a pacemaker in his chest. He has been having difficulty breathing, tiredness, chest pains ect.. i took him in to see a doctor at the V.A Clinic which he usually goes to but everytime he went in there they just send him back home.. And the symptoms are doig nothing but getting worse.. can you tell me why this migh be happening or tell me what i should do?
