How To Cure Severe Stomach Pain And Dark Stools Post Hysterectomy?

I have had 13 abdominal surgeries caused from hysterectomy to adhesions to many blockages and resections until 4 years ago had 97% of colon removed and since have constant diareah and recently have had very dark stool, liquidy and bubbly and now have severe pain in stomach and blood when I wipe after going bm. No straining or pain rectally. Do I need to see dr? What could it be?
