How To Deal With Throat Issues With Shoulder Pain?

hi,my dad is having bad throat from 2 weeks,also area around his shoulder is paining.His digestion is not proper ,he does't feel hungry,in side his mouth theres small dots(in hindi its called funsi).behind his ear there's small round shaped muscle which has increased from last yr.He already has gouty artheritis and has stone in his right kidney.To which department should i take him. pls reply soon
