How To Fade The Scar Of Surgery?

Hi there! I have had my third open heart surgery a bit over a year ago when I was 19. My scar got cleaned up and was nice and thin, but now it is flattening, light pink and getting wider. Is there a way I can get it back to a thinner look or fade it all together?
