How To Treat Infection Due To Stitches In Hand?

My boyfriend got stiches on his hand on Saturday. It appears that his hand is now getting infected. He is taking antibiotics, Tylenol and pain meds. Should we go back to the Dr? He contacted the nurse yesterday and was told to add warm compress to hand, so that the fluid can go down. His middle finger and ring finger appears to have a lot of fluid.
