How To Treat Large External Hemorrhoids?

My husband has rather large external hemorrhoids. I was looking at them tonight and noticed the big one on the left side had these little white bits just under the tissue. They look like small grains of rice. I was able to gently squeeze one out and it looks like kind of thick pus. I have "checked on" his hemorrhoids before and never seen anything like this. He hasn't reported anything unusual. What might this be and what should he do about it?
