How To Treat Leakage From Penis?

Dear sir,
Am a 22 yr old male from jaipur india...M here for a solution to my question/doubt regarding fluid leakage from penis....I started masturbating when i was 14...And the duration is 2-3 times in a week....the problem is since last 3 years my penis is leaking semen/fluid the day long even when am not sexually aroused....a foul smell comes all day from the area....even when i clean my belly i can see a pulpy material coming out my penis and when it comes the next moment i feel bad....really sir/ma'am i think there is something worng in the system..however there is no difficulty while masturbation...i feel low esteemed these days....m having sever hairfall....changes in face...disturbed life style...please help me...
