How To Treat Lesions On Forehead Causing Hair Loss?

I have some lesions on my forehead that won't heal some in my hair . I've had hair loss some of my pores will look dark I can mash and something comes out like a small splinter sometimes they will be laying on the skin the lesions have many white round balls and the little black splinters and black spots if you don't get all the white balls out the lesion will not get better it will heal over then it will become inflamed and bust back open I can take tweezers pull out balls and they have like a tail on them when removed it leaves a hole please help I have been going through this for a year and its embarassing
