How To Treat Stiffness Of Neck And Tightness Around Shoulder?

I got a stiff neck about four days ago. I havnt changed my sleeping arrangments at all and have not done anything out of the ordinary. Despite heat, message, neck and shoulder rotation, mild pain relievers and muscle relaxers it is getting more and more painful. Today it moved down into my shoulder and is causing cramps and pulling my shoulder up towards my ear.
