Husband Has Had Extreme Bleeding Off And On During Bowel

Husband has had extreme bleeding off and on during bowel movements for almost 3 years. There are times where there is no stool, just streaming blood. He has had to have a blood transfusion because of it. He has had a colonoscopy, and the only thing found was a few hemorrhoids, but nothing to explain the extreme pain in his upper left and left side abdomen or the blood. He also has extreme fatigue and constant brain fog.
We do not have insurance, so seeing a gastrointerologist is not an option at this time. Any suggestions?
