I Am 38 I Had A Ct On My Chest

I am 38 I had a ct on my chest which revealed multiple lung nodules, pleural effusion and enlarged lymph node. I had a resection of my right lung about 7 years ago which showed a necrotic caseating granuloma. I don't know what it could be that's causing all this, do you have any ideas? My doctor wants to wait three months and do another ct scan but if it is valley fever again shouldn't I start the anti- fungals now?
