I Am 43 Years Old, Have Not Had A Normal

I am 43 years old, have not had a normal period in about 7 months. Last month I spotted very lightly one day and today I've had a couple of light brown spots but I'm having debilitating cramps and sharp pains inside my vagina, headaches, and nausea. Can you please tell me what this could be?
