I Am 74 Years Old, With Glucose Level Of 151

I am 74 years old, with glucose level of 151 and BUN of 24 and creatinine of 1.03. I also have tingling and numbness in lower legs and pain in both feet just below toes. What should I tell my new family doctor? My grandfather (maternal) was diabetic and died from it.
