I Am A 45yo Female With Prior History Of Renal

I am a 45yo female with prior history of renal stones. I underwent lithotripsy at ages 23 and 40 and have passed stones on my own. Saw my dr today because I have started suffering that familiar flank pain. CT Urogram shows 78 mm stone in the lower pole of my right kidney along with an extrarenal pelvis. I'm told the stone is not obstructive but the extrarenal pelvis is a new finding. I had the same test 13 months ago with no mention. Should I be concerned that there is an obstruction?
